About Us
The Heart Health Leaders Network (HHLN) is a consortium of leaders helming from some of the nation’s most innovative and forward-thinking health systems who seek to improve cardiovascular care delivery and close the gap between guidelines and implementation. The HHLN’s mission is to promote awareness of cardiovascular care as a critical population health priority and drive the development of industry-supported partnerships to accelerate the adoption of appropriate, guideline-driven care. The network aims to:
Provide a networking forum for cardiovascular care experts to facilitate data and best practice sharing
Identify opportunities for cardiovascular experts to collaborate on efforts to refine care pathways and promote care model innovation
Participate in network and industry consensus building
Design and implement real world evidence (RWE) and/or pilot programs
